1. Medical Disclaimer

All services at Balance Facial Aesthetics in Baton Rouge, LA are elective cosmetic procedures. Results vary by individual and are not guaranteed.

2. Informed Consent for Aesthetic Treatments

By receiving treatment, you acknowledge that you have been informed of risks, benefits, and alternatives and give voluntary consent.

 

 

Neurotoxin, Tox, Toxin, Neuromodulators (e.g., Botox/Dysport/Xeomin/Daxify/Jeuveau)

 

 

You understand:

 

• Results are temporary (typically 3–4 months)

• Risks include bruising, asymmetry, ptosis, headache, and rare systemic effects

 

 

 

Dermal Fillers (Versa, Belotero, Juvederm, Restylane, RHA) 

 

 

You understand:

 

• Results vary and may require multiple treatments

• Risks include swelling, bruising, nodules, asymmetry, and vascular occlusion, which may lead to tissue damage or blindness (rare but serious)

 

 

 

Chemical Peels

 

 

You understand:

 

• Peeling, redness, and sensitivity are expected

• Risks include hyperpigmentation, irritation, or scarring

 

 

 

Microneedling

 

 

You understand:

 

• Multiple sessions may be required

• Risks include redness, pinpoint bleeding, infection, and irritation

 

 

You agree to follow all pre- and post-care instructions.

 

GLP-1 Medication Terms & Conditions

By purchasing or participating in our GLP-1 weight management program, you acknowledge and agree to the following:

• GLP-1 medications require a medical evaluation and approval by a licensed healthcare provider. Eligibility is not guaranteed.

• Results vary by individual. No specific weight loss or health outcomes are promised or guaranteed.

• You must provide accurate and complete medical information. Failure to do so may affect your eligibility and safety.

• It is your responsibility to follow all prescribed dosing instructions and attend recommended follow-up appointments.

• GLP-1 medications may cause side effects. You should promptly report any concerning symptoms to your healthcare provider.

• Program fees, consultation fees, and medication costs are subject to our published payment and refund policies. Unless otherwise stated, fees for services already provided are non-refundable.

• Medication availability may be affected by manufacturer supply or pharmacy inventory and cannot be guaranteed.

• This program is not a substitute for emergency medical care or ongoing primary healthcare.

• By participating, you consent to electronic communication regarding your treatment, appointments, and program updates, in accordance with our Privacy Policy.

• By enrolling, you confirm that you have read, understood, and agree to these Terms & Conditions.

 

 

3. Eligibility

 

 

You must be 18 years or older unless accompanied by a parent/legal guardian.

 

You agree to disclose all relevant medical history, medications, allergies, and prior treatments.

 

 

 

 

4. Appointments

 

 

By booking, you agree to:

 

• Provide accurate information

• Arrive on time

• Follow clinic policies

 

 

 

 

 

5. Cancellation Policy

 

 

• 24-hour notice required

• Late cancellations/no-shows: $25

• Repeat no-shows may require prepayment

 

 

 

 

 

6. Late Arrivals

 

 

Arrivals over 10–15 minutes late may be rescheduled and charged a fee.

 

 

 

 

7. Payments & Refunds

 

 

• Payment due at time of service

• All sales are final

• No refunds on services rendered

• Packages are non-transferable and may expire

 

 

 

 

 

8. Financing Policy

 

 

We may offer third-party financing options (e.g., Cherry, CareCredit).

 

By selecting financing:

 

• You agree to the lender’s terms

• Balance Facial Aesthetics is not responsible for financing approvals, interest rates, or payment disputes

• All payments remain your responsibility

 

 

 

 

 

9. Photo & Media Consent

 

 

By receiving treatment, you consent to photography/video for:

 

• Medical documentation

• Education

• Marketing (optional—can be declined)

 

 

If you consent to marketing use:

 

• Images may be used on our website, social media, or advertising

• No identifying information will be shared without additional consent

 

 

You may withdraw consent in writing at any time.

• Complications when aftercare is not followed
